Community VNA Locations Are Now Active Day Health Care Centers
TREVOSE, PA – January 27, 2021 – Active Day is pleased to announce the acquisition of three Adult Day Health Care centers from Community VNA (Attleboro, MA). The acquired centers are located in Taunton, Norwood, and Mansfield and will join existing Active Day centers in Hingham, New Bedford, Fairhaven, and Lowell.

Healthcare services are at the core of the adult day experience and include the following.
– Registered Nurses for physical and mental assessments, to oversee chronic conditions and provide medication management.
– Personal Care Assistance for participants who require extra help with activities of daily living.
– Social Workers to meet with participants and their families to understand their needs and advocate, and offer guidance and caregiver support.
– Family Caregivers Assistance through access to social workers to resolve family concerns, handicapped accessible transportation, and personal care and hygiene.
The centers have re-opened as Active Day of Taunton, Active Day of Norwood, and Active Day of Mansfield. In addition to adult day health centers, Active Day provides home care services throughout the South Shore region.
